Reporting Forms for Independent Schools

Required Reports

The Commission on Independent Schools requires that all member schools submit the following reports. Due dates will be noted in letters sent to schools following Commission actions as noted. Schools will be emailed a reminder letter with the appropriate form attached prior to the report due date. 

  • Two-Year Progress Report
    Due following Commission action on the evaluation visit.
     
  • Five-Year Progress Report 
    Due following Commission action on the Two-Year Report.

Other Information/Reports

Additional reports or information may be requested from a member school under certain circumstances, for example:

  • when a school undergoes a substantive change (Substantive Change Report)
     
  • when two or more schools merge (Report for Merged Schools)
     
  • when a school is required to respond to specific concerns expressed and identified by the Commission (Special Progress Report)
     

Examples of substantive changes include adopting a new mission, purpose, or admitting different students, adding a division, adding a residential or homestay program, adding or removing enrollment of a sex, adding a new local campus that will serve current students, and changing ownership for a proprietary school. The merging of two or more schools is also a substantive change but is addressed by a separate procedure and report. 

A school should notify NEASC of substantive changes or school mergers in the Annual Report or at any time throughout the year by contacting Commission staff at cis@neasc.org.
